|Dependency Override Appeal (Size: 202 KB)||The Department of Education does allow financial aid administrators to use professional judgment in cases where extenuating circumstances prevent a student from being able to provide parental information on the Free Application for Federal Student Aid (FAFSA), commonly referred to as a dependency status appeal or override.|
|Financial Aid Appeal Form (Size: 93 KB)||This form is used to appeal a determination that you have not maintained Satisfactory Academic Progress (SAP).|
|Income Tax Information (Student or Parent)-Order a Transcript||If you were selected for verification by the Department of Education, you must provide a copy of your tax information. Please check your student account to determine for which tax year information is needed.|
|Orphan/Ward Of The Court/Foster Care Form (Fillable, Size: 163 KB)||Your status as an independent student for financial aid purposes was based upon your answer to questions 52, 53, or 54 on the Free Application for Federal Student Aid (FAFSA).|
|Professional Judgment Appeal (Size: 202 KB)||The Financial Aid Office can use their professional judgment to make adjustments on your Student Aid Report (SAR) because of unusual circumstances. You will have to provide enough documentation to support any adjustments.|
|Unaccompanied Homeless Youth Form (Size: 155 KB)||For students who have certified on the 2015-2016 Free Application for Federal Student Aid (FAFSA) that he/she is an unaccompanied homeless youth or a youth at risk of being homeless.|
|Verification of Low Income (Size: 170 KB)||This form is for students who reported income for yourself and/or your family on your 2015-2016 Free Application for Federal Student Aid (FAFSA) appears insufficient to support the number of people in your household. Please complete this form to clarify how you and/or your family were able to provide for such needs as housing, food and utility bills during 2014 so that we may properly complete your financial aid award.|
